The algorithm suggests only 2 possible outcomes: PEA/asystole or Vfib. Although it seems intuitive, I'd suggest a branch that leads to ROSC and then to the post-resuscitation algorithm.

Where closed chest CPCR is ineffectve (e.g due to massive haemo/pneumothorax) can the algorithm be modified to identify when open chest CPCR is indicated and must be initated immediately. There is after all no point going through these cycles if you are unable to create a decent cardiac output....

Further to Nathan's point, where does pulseless VT fit into the algorithm? in the guidelines it suggests de-fib but this is not clear in the algorithm.
